Marriage, Divorce and Separated Case

• If the applicant is married or for the addition of Spouse's Name in the Passport :
1. MARRIAGE CERTIFICATE or
2. Joint Photo Declaration signed by both husband and wife as per Annexure 'J'.
• For deletion of Spouse Name :
1. Divorce Order/Decree.
• For Change of Spouse Name :
1. Divorce Order/Decree or Death Certificate of first spouse's name and
2. Re-marriage certificate or Joint Photo Declaration signed by both husband and wife as per Annexure 'J'.
• For a change of surname by women applicants following marriage/divorce:
1. Marriage Certificate or Joint Photo Declaration signed by both husband and wife as per Annexure 'J'.
2. Divorce Order/Decree (if name/surname change is based on divorce).
• In case of a full change of name after marriage, the standard name change procedure should be followed.

Minor

• It is assumed that consent of both parents is available, unless specified.
• For minor applicants, present address proof document in the name of parent(s) can be submitted.
• It is advised to carry original and self-attested copies of parents passport to Passport Seva Kendra (PSK), in case parents possess passport.
• For minor applicants, documents can be attested by parents.
